Количественная оценка ДНК вирусов папилломы человека высокого канцерогенного риска и герпесвирусов человека у мужчин при нарушении фертильности

Аннотация

Бесплодие является актуальной медицинской и социальной проблемой, у 50% пар оно связано с мужским фактором, и в половине случаев этиология его остается неизвестной. Цель работы состояла в изучении распространения и количественном анализе герпесвирусов человека (ГВЧ) и папилломавирусов высокого канцерогенного риска (ВПЧ ВКР) у мужчин с бесплодием, а также в оценке влияния этих инфекций на показатели спермы. исследовали образцы эякулятов 196 мужчин, разделенных на 3 группы. в 1-ю группу вошли мужчины с бесплодием неясной этиологии (n = 112), во 2-ю группу – пациенты, у жен которых были в анамнезе зафиксированы случаи самопроизвольного прерывания беременности (n = 63), в 3-ю группу (контрольную) – здоровые мужчины, обратившиеся для профилактического обследования (n = 21). Количественный анализ ДНК ГВЧ (цитомегаловирус, вирус Эпштейна–барр и вирус герпеса человека 6) и ВПЧ ВКР 12 типов, относящихся к клайдам А5/6, А7 и А9, проводили методом полимеразной цепной реакции в реальном времени, спермиологическое исследование осуществляли согласно рекомендациям ВОЗ. ДНК ГВЧ и ВПЧ ВКР в эякуляте обнаружены суммарно у 42/196 (21,4%) мужчин: у 31 и 11 пациентов в 1-й и 2-й группе соответственно (p > 0,05). ни у одного из здоровых мужчин вирусная ДнК в эякуляте не была обнаружена. ГВЧ выявили у 24/42, ВПЧ ВКР – у 18/42 мужчин (p > 0,05) без значимых различий между группами. Среди вПЧ вКР в эякулятах чаще встречались генотипы клайда А9 (14/18; р = 0,04). Сравнительный анализ спермограмм показал, что у инфицированных пациентов были значительно снижены подвижность, а также количество морфологически нормальных сперматозоидов по сравнению со здоровыми мужчинами. При количественном определении вирусных ДНК обнаружено, что в эякулятах 31% мужчин вирусная нагрузка была > 3 lg10 на 100 тыс. клеток. Обнаружение ГВЧ и ВПЧ ВКР в эякулятах ассоциируется с мужским бесплодием, и количественное определение вирусных ДНК в эякулятах является полезным индикатором для мониторинга вирусных инфекций при бесплодии и своевременного принятия решения о начале терапии.

Abstract

Infertility is an actual medical and social problem. In 50% of couples it is associated with the male factor and in more than 50% of cases the etiology of the infertility remains insufficiently understood. The goal of this work was to study the prevalence and to perform quantitative analysis of the human herpes viruses (HHV) and high carcinogenic risk papilloma viruses (HR HPV) in males with infertility, as well as to assess the impact of these infections on sperm parameters.

Ejaculate samples obtained from 196 males fall into 3 groups. Group 1 included men with the infertility of unknown etiology (n = 112); group 2, patients who had female partners with the history of spontaneous abortion (n = 63); group 3 (control), healthy men (n = 21).

HHV and HR HPV DNA in the ejaculates were detected in a total of 42/196 (21.4%) males: in 31 and 11 patients in groups 1 and 2, respectively (p > 0.05) and in none of healthy males. HHV were detected in 24/42; HR HPV, in 18/42 males (p > 0.05) without significant difference between the groups. Among HR HPV genotypes of the clade a9 in ejaculate were more frequent (14/18, p = 0.04). comparative analysis of the sperm parameters showed that in the ejaculates of the infected patients sperm motility as well as the number of morphologically normal cells were significantly reduced compared with the healthy men. The quantification of the viral DNA revealed that in 31% of the male ejaculates the viral load was high: >3 lg10/100000 cells.

Conclusion. The detection of HHV and HR HPV in the ejaculate is associated with male infertility. Quantification of the viral DNA in the ejaculate is a useful indicator for monitoring viral infections in infertility and for decision to start therapy.
